Download a copy of the enrolment form and submit it with payment to CSTC either via email, post or fax.

*Payment must accompany your course enrolment in order for CSTC to process your application and to confirm your position on the course

Payment options are provided on the reverse side of the enrolment form.

A confirmation letter will be sent to the participant of the course to confirm the booking.  Please ensure that you read the confirmation letter carefully as it contains important information about the course including dates/times, pre-requisites and special requirements specific to the course.
